Age-related cognitive changes and preventing memory loss

It is common for the ageing brain to take longer to process the information which might result in forgetfulness and word finding difficulties. These changes may impact an individual's function. The memory changes cause stress and anxiety which leads into a vicious cycle, making stress and anxiety worse.

It is helpful to see a geriatrician who can do a cognitive assessment, which includes a baseline cognitive testing and a comprehensive review of other systems that can impact on memory which include mood, sleep, medical conditions, and medications. Management will focus on managing any modifiable factors contributing to memory changes. The baseline cognitive testing will assist if things change in the future.

Dementia Management

During your consultation, the geriatrician will discuss with you about cognitive enhancers that can slow down the early signs of dementia depending on the type of dementia. Commencing these medications in the early stages of the disease is more beneficial than waiting for the disease to advance which impacts on living at home independently.

Dementia affects people in different ways. As the dementia progresses, your loved ones might develop change in behaviours which can include anxiety, agitation, aggression, hallucinations, loss of inhibition and depression. Our Geriatrician will help in creating a behaviour support plan to manage these behaviours of concern and assisting the carers in managing their loved ones with dementia.

Comprehensive geriatric Assessment evaluates various aspects of an older individual’s health, including physical, social, and mental well-being. This systematic evaluation allows for multiple issues to be identified and addressed accordingly. Importantly it appreciates how certain medical conditions, medications, and social aspects of a person’s life can impact on their health. The patient’s goals and wishes remain the centre of the assessment and a collaborative management plan is created by your geriatrician.

Falls Prevention

Falls are a major health concern for older people.  Falls are the leading cause of unintentional injury in older Australians. Falls are a major cause of admission to hospital and unfortunately for some people a severe fall may make it difficult to return back home and maintain their independence. A comprehensive falls assessment will review your risk of falling and will provide you with a plan to decrease the chance of having a fall. Falls assessment will look at any modifiable risk factors in preventing falls. The Geriatrician will also assess your bone health to prevent major fractures.

Polypharmacy is defined as the use of 5 or more medicines of any type including over the counter and natural therapies. People who take many medications will benefit from a medication review to reduce the risks associated with polypharmacy. If you are concerned about the number of medications you take, talk to your GP for a referral to a geriatrician. Age Well Specialist Centre provides the high quality, comprehensive and personalised medication assessment.

Medication’s Review

Incontinence describes any accidental or involuntary loss of urine from the bladder known as urinary incontinence or faeces known as faecal incontinence. Incontinence can range in severity from a small leak to a complete loss of bladder or bowel control. Incontinence can significantly impact an older person’s quality of life but help is available. If you or family member suffers from this condition you will benefit from a comprehensive assessment by a geriatrician.
